INSTRUCTIONS FOR OBTAINING A CHILD PASSPORT
(age 16 or under)



Federal Law requires infants and children to carry individual Passports. (Children may no longer be included in the parent's passport)

1. One (1) application form (DS-11), available from Post Office or County Clerk's office (or you may click here. *DO NOT sign the application until requested to do so by the acceptance agent at the Post Office or County Clerk's office. Children ages 13 - 15 must appear with a parent.

2. Two (2) identical Passport size photos. *NOTE: Passport photos of infants should include the entire baby's body. (call our office with any questions.)

3. Submit child's previous passport (if available) or Original Certified Birth Certificate. (Our office can obtain one for you in 1-4 working days). This Birth Certificate must contain the following: Original State or Coutny Seal, Filing Date and Registrar's signature. NO XEROX COPIES, HOSPITAL COPIES OR NOTARIZED COPIES WILL BE ACCEPTED.

4. 1 Check or Money Order Payable to "Department of State" in the amount of $120.00. The $120.00 CHECK MUST BE PLACED INSIDE THE SEALED ENVELOPE. A second check for $25.00 payable to the post office or county clerks office.

5. Proof of Identity. The parent will present their valid drivers license from any state with photo. If last name on license differs from child's, law requres presentation of legal documents for proof of parentage. *NOTE: your drivers license must have been issued over one (1) year ago. If your license has been issued in less than a year, or if you can not prove your identity you will be requred to issue other documents. *NO TEMPORARY OR LEARNERS LICENSE ACCEPTED.

6. Proof of Departure - Copy of Airline Tickets or Travel Itinerary (only one is required if you are leaving in the next 3 weeks).

7. Letter signed by the parent and the child (if the child is able to sign), if not, the parent can print the child's name on the letter, then the parent will sign the child's name. The letter must be inside sealed envelope. (Click here to download child letter and here for the adult release letter)

8. To Expedite - Federal law requires the applicant's parent/either one, to appear in person with items 1-7 listed above, to your local acceptance agent for the passport applications to be executed. Ask the clerk to process the documents and place in a sealed envelope addressed to "Passport Services Houston, Texas". You must request the sealed envelope back. Advise them that you have someone personally walking the passport application to the passport office. *NOTE: The release letter and proof of departure must be included inside the sealed envelope. If these are missing te passport will be sent back to the address on the application usually within 4-7 days. the sealed envelope must be presented to our office within three (3) working days.

9. Be sure to include the Order Form, filled out and paper clipped to the outside of your sealed envelope. Be sure to list your first and last name, address where you want your document returned, phone number in case we need to contact you. Be sure to also list your birth date. *NOTE: your address must have a street address as most overnight services will not deliver to PO Box. Validity of Passport - 5 years.
