1. Completed Green application form (DS-4085). Click here to download the file in .pdf format (or, if you choose, you may obtain this form from the Post Office or County Clerk's office). To download this form, you must have Adobe® Reader® installed on your computer. If you do not, please click here. 2. Submit your valid US passport. 3. Governments fee of $60.00, if your extra pages are needed within 3 weeks. If your pages are needed in no rush there is no Government Fee. 4. Release letter. Click here for a sample adult release letter, and click here for a sample child release letter. 5. Proof of departure, if your pages are needed within 3 weeks (only 1 of the following 3 is needed) a) Copy of round trip airline tickets b) Company letter of Expedite from your company (click here) c) Travel itinerary 6.
