INSTRUCTIONS FOR OBTAINING A NEW PASSPORT



1. One (1) application form (DS-11), available from Post Office or County Clerk's office or from the US Consular Affairs website.

If you are picking up a printed copy at the post office or couty clerk's office, DO NOT sign the application until requested to do so by the acceptance agent at the Post Office or County Clerk's office.

2. Two (2) identical Passport size photos. *NOTE: Passport photos of infants should include the entire baby's body. (call our office with any questions.)

3. Original Certified Birth Certificate issued by the state or county in which you were born. Birth Certificate must have a file number and registrar’s signature. (you will get your birth certificate back with the completed passport). If you do not have the original Birth Certificate see our website www.texastower.net If you have an expired passport you can use that in place of the birth certificate. (If you are a Naturalized Citizen you must submit your original Naturalization Certificate, you will get your Naturalization Certificate back with the completed passport). *No xerox copies, hospital copies or notarized copes will be accepted by the passport office.

4. 1 Check or Money Order Payable to “Department of State” in the amount of $135.00. The $135.00 check must be placed inside the sealed envelope. A second check for $25.00 payable to the post office or county clerks office.

5. Proof of Identity: You must present to the acceptance agent your valid driver’s license. Your driver’s license must have your photo on it and must have been issued over 6 months ago. If your driver’s license has been issued in the past 6 months, OR if you can not prove your identity you will be required to issue other documents, please contact our office. *No temporary licenses. The acceptance agent will look at your license and give it back to you. If your name has changed please include original documents showing how your name changed (Marriage certificate or court documents).

6. Proof of departure, if your pages are needed within 3 weeks (only 1 of the following 3 is needed)
a) Copy of round trip airline tickets
b) Company letter of Expedite from your company (click here)
c) Travel itinerary

7. Release Letter authorizing our office to collect the passport on your behalf. This release letter must be signed and dated by the original applicant. *The release letter must be in the sealed envelope! Be sure to give it to the agent at the Post Office or Country Clerk’s Office when you go to make your personal appearance. (click here)

8. To expedite: Take the above requirements to your local acceptance agent for passport applications to be executed (usually a special post office or, in some cases, a County Clerk’s office) Ask the clerk to process the documents and place them in a “Sealed Envelope” addressed to “Passport Services”. You must request the sealed envelope back. DO NOT LET THE AGENT KEEP IT! Please advise them that you are having someone personally walk the passport application through the passport office for you. NOTE: The release letter and proof of departure must be included inside the sealed envelope. *Sealed envelopes received by our office lacking proof of departure and/or release letters will be sent back to the address on the application. The sealed envelope must be presented to our office within three (3) business days.

9. THE FOLLOWING IS FOR INDIVIDUALS THAT ARE NOT DROPPING OFF THEIR INFORMATION IN PERSON Be sure to include the “Order Form” (click here) filled out and paper clipped to the outside of your sealed envelope.
