REQUIREMENTS
FOR PASSPORT RENEWAL
(Adults only Age 16 or older)
1.Completed Red application form (DS-82). This form may be obtained
from the Post Office or County Clerk's office or from the US Consular
Affairs website.

If you are picking
up a printed copy at the post office or couty clerk's office, DO
NOT sign the application until requested to do so by the acceptance
agent at the Post Office or County Clerk's office.
2. Two (2) identitical passport photos.
3. Submit your expired US passport. *Your passport must have been
issued within the past twelve (12) years. *Alert: the passport office
can renew the passport only if it has been issued within the past twelve
(12) years, not from the expiration date, the issue date only. Please
check your passport.
4. Department of State Fee is $135.00.
5. Release letter (click here).
This letter allows the passport office to release your passport to our
office. Be sure to sign and date it. 6. 6. Proof of departure
in the form of: (only 1 of the following is required)
a) Copy of round trip airline ticket.
b) Company letter of Expedite from your company (click here)
c) Travel itinerary
NOTE: If you do not have travel plans and are in no hurry to get your
passport, the Passport will be issued according to the date on your application
form.
*NOTE: If the expired passport was issued more than twelve (12) years
ago, the applicant had not reached his 16th birthday when the expired
passport was issued, please follow instructions for "New Adult Passport".
You can use the expired passport in place of your birth certificate.
